During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.
New Patient Registration
To streamline patient visits, we require all New Patients to complete their registration online before their appointment.
After scheduling your appointment by phone, you will receive an email with a secure registration link sent to the email address provided.
This link will expire after 72 hours, so please complete your registration as soon as possible.
Failure to complete your registration before your appointment may result in your appointment being cancelled/rescheduled.

What to Bring to Your Appointment
- Valid Photo ID
- Insurance Card(s)
- Name of your current Doctors (Examples: PCP & Cardiologist)
- List of all current allergies and medications
- Recent medical records or x-rays will be helpful if you are seeking a second opinion or transferring your care
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
